When the countermeasures are off target, the issues return. If issues reoccur for the same source, we have not actually addressed the trouble. We have only temporarily contained it. Even if the exact same issue persists as a result of a various source, it is feasible that trouble addressing was done without a complete sufficient root cause evaluation action. Issue fixing has not efficiently occurred until we could validate that the root causes have been determined and that the countermeasures put on them are effective.
